At Sierra at Southlake, we understand that navigating the admissions process alone can be challenging. Our admissions team and a streamlined process can help you receive the care you need without delay.
Our Admissions Criteria
Sierra at Southlake provides treatment for adults age 18 and older of all genders who are struggling with a mental health disorder. We treat a range of behavioral health concerns, including:
- Trauma
- Posttraumatic stress disorder (PTSD)
- Anxiety
- Depression
- Bipolar disorder
We also provide treatment for adults who are struggling with co-occurring mental health conditions. Based on our criteria, our admissions team can help you determine if our outpatient treatment center is right for you.
The Admissions Process
The goal for our admissions process is to gather the information we need to determine if Sierra at Southlake is right for you and which program will be most beneficial.
To begin the process, clients have a phone call with our admissions team. Through a detailed assessment, our admissions team will collect information about your:
- Mental health history
- Current mental state
- Treatment history
- Goals for treatment
Based on the information gathered, our team helps each adult determine if our intensive outpatient program (IOP) or partial hospitalization program (PHP) will be the most beneficial for them.
Our admissions team is available 24/7 to answer any questions you might have about the process, our programs and your insurance coverage. If we decide that a different outpatient treatment center can better meet your needs, we will make the appropriate referral.
Starting Your Sierra at Southlake Experience
After determining that Sierra at Southlake is right for you, we will begin creating your customized treatment plan for either our IOP or PHP.
Depending on which program you will be participating in, your care team will work with you to determine which services will be included in your treatment plan. As part of your treatment plan, you will also receive personalized aftercare planning. We begin to develop your aftercare planning during admissions so that you know what your next steps will be after completing our program.
